If any information in your 'Cattle on Holding' list is incorrect, you can use this box to tell BCMS what is wrong. Please provide information for individual animals on the View animal details and history page.

If you need a continuation sheet for a cattle passport, please enter the animal’s eartag number.
Use this page if you need an application form and guidance notes for a DNA test. You should only use this form for an appeal when a cattle passport has been refused because of a late application. (You do not need a form to carry out a DNA test in other circumstances).
Samples must be taken from the calf and its dam by a vet and processed by an approved laboratory.
Please enter the eartag numbers of the calf and dam, and the calf’s date of birth.
You must also confirm that the dam is still alive, and give the holding number where she is now. If you are not the keeper of the dam, you will need the current keeper’s permission to carry out the test. If you do not know the location of the dam, BCMS cannot give you this information and will not issue a test pack.
Form CPP16 is used to apply for passports for cattle that you have imported from outside Great Britain (GB). Please tell us how many imported animals you wish to register.
If one or more of your cattle passports have been lost, stolen or destroyed, BCMS can send you a form to apply for replacements. Please enter the eartag number(s) of the animal(s).
There is a charge of £20 for each passport, to be paid when you return the completed form to BCMS.
Please state what type of pre-paid envelope you need, for example small (for movement cards) or large (for pre-printed passport application forms). Please also state how many you need.
